29 CFR 1915.12(d)(2)(ii): The employer did not ensure that each employee entering a confined space, enclosed space, or other areas with dangerous atmospheres was trained to anticipate and be aware of the hazards that may be faced during the entry. Fish Tank: On or about February 10, 2015 and times prior thereto, employees would enter the tanks to clean with corrosive sanitizing cleaners. Depending on the weather if it was bad weather the hatches would be closed. In this condition employees could be overcome from oxygen deficient or corrosive contaminates hazard.

29 CFR 1915.77(c): When employees are working aloft, or elsewhere at elevations more than 5 feet above a solid surface, either scaffolds or a sloping ladder, meeting the requirements of this subpart, shall be used to afford safe footing, or the employees shall be protected by safety belts and lifelines meeting the requirements of ���� 1915.159 and 1915.160. Employees visually restricted by blasting hoods, welding helmets, and burning goggles shall work from scaffolds, not from ladders, except for the initial and final welding or burning operation to start or complete a job, such as the erection and dismantling of hung scaffolding, or other similar, nonrepetitive jobs of brief duration.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: Factory: On or about February 10, 2015 and times prior thereto, employees were using corrosive cleaners without a properly operating eyewash in the area. This exposed employees to eye injuries from not having suitable flushing capabilities for immediate use.
