29 CFR 1926.100(a):   Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a) Ground Floor - Front Wall: CSHO observed employees without the use of protective helmets exposed to struck by hazards while stacking framing wood below framing activities, on or about 2/24/15.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) Top of plate 2nd floor - front wall: Employee without any means of fall protection was exposed to a fall hazard of approximately 20 feet while engaged in framing activities , on or about 2/24/15.

29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.   a) Top of Plate 2nd floor - Front Wall: Employee without any eye protection was engaged in framing activities using a sawzall , on or about 2/24/15.   Frame Q LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.95(a), which was contained in OSHA inspection number 979064, citation number 01, item number 01 and was affirmed as a final order on 7/22/14,  with respect to a workplace located at 18 Skyline Dr., Englewood Cliffs NJ.
