29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:   a) 332 Applegarth Rd., Monroe township, NJ site - Facility:    The temporary agency employer did not develop, document and certify a hazard assessment to identify potential hazards temporary employees would be exposed to. Two temporary employees were hired to work at Bulk Express Logistics where they were exposed to hazards associated with operations such as but not limited to forklift operation without training and various chemical hazards.  A written certification of the hazard assessment was not documented.   Condition noted on or about 12/10/2014.
