Box 304, Troy, PA - Employees used a double-bridle sling to lift logs that had a makeshift shortened leg, created by placing a hook on a lifting attachment through a link of an alloy steel chain. Employees were exposed to struck-by hazards due to the unknown rated capacity of the sling, as observed on or about February 25, 2015.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

Box 304, Troy, PA: Employees used a double-bridle sling daily to lift logs with alloy steel chains that lacked permanently affixed durable identification (stating size, grade and rated capacity). Employees were exposed to struck-by hazards due to the unknown rated capacity of the sling, as observed on or about February 25, 2015.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.184(e)(3)(i): A thorough periodic inspection of alloy steel chain slings was not made on a regular basis and/or were done at intervals greater than once every 12 months:  a) Slicer Area, 23189 S.R. 14N, P.O. Box 304, Troy, PA: Employees used a double-bridle sling daily to lift logs with sling components that were not regularly or periodically inspected for damage. Employees were exposed to struck-by hazards created by possible damage going undetected, as observed on or about February 25, 2015.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Debarking Area, 23189 S.R. 14N, P.O. Box 304, Troy, PA - There were no means to restrict access to employees that worked and walked adjacent to the rotating parts of the HMC debarker. Employees were exposed to caught-in hazards, on or about February 25, 1015.    In accordance with 29 CFR 1903.19(c), abatement certification is not required for this violation. The hazardous condition was corrected during the inspection.    Oak Hill Veneer, Incorporated was previously cited for a violation of this occupational safety and health standard which was contained in OSHA Inspection Number314164146, Citation Number 1, Item Number 5 issued on August 24, 2011, and was affirmed as a Final Order on September 21, 2011, with respect to the workplace located at 23189 S.R. 14N, P.O. Box 304, Troy, PA.

Box 304, Troy, PA - Employees used compressed air at 65 p.s.i. to clean wood chips from work areas and machines. Employees were exposed to struck-by hazards caused by high pressure air, as observed on or about February 25, 2015.    In accordance with 29 CFR 1903.19(c), abatement certification is not required for this violation. The hazardous condition was corrected during the inspection.    Oak Hill Veneer, Incorporated was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard, 1910.242(b) which was contained in OSHA Inspection Number 618059, Citation Number 1, Item Number 1 issued on November 13, 2012 and was affirmed as a Final Order on December 20, 2012, with respect to the workplace located at 23189 S.R. 14N, P.O. Box 304, Troy, PA.    Oak Hill Veneer, Incorporated was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1910.242(b) which was contained in OSHA Inspection Number 314164146, Citation Number 1, Item Number 8 issued on August 24, 2011 and was affirmed as a Final Order on September 21, 2011, with respect to the workplace located at 23189 S.R. 14N, P.O. Box 304, Troy, PA - An Ansul ABC dry chemical fire extinguisher provided for employee use to fight fires was placed on the floor and not mounted at its identified location. Employees looking to fight incipient stage fires in the area were exposed to the slip, trip, and fall hazards and to the hazards associated with fires and explosions, on or about February 25, 2015.  a) Slicer Area, 23189 S.R. 14N, P.O. Box 304, Troy, PA - An Ansul carbon dioxide fire extinguisher provided for employee use to fight fires was placed underneath a desk and not mounted at its identified location. Employees looking to fight incipient stage fires in the area were exposed to the hazards associated with fires and explosions, on or about February 25, 2015.  In accordance with 29 CFR 1903.19(c), abatement certification is not required for this violation. The hazardous conditions were corrected during the inspection.
