29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about March 2, 2015 during the installation of roofing materials, the employer did not ensure that employees working on the ground were protected from head injury by falling material by providing and requiring the use of protective helmets.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   On or about March 2, 2015, during the installation of roofing materials at a newly constructed residential home, the employer did not ensure employees were protected from potential eye injuries while using air powered roofing nail guns.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about March 2, 2015 during the installation of roofing materials on a residential home, the employer did not ensure employees were protected from a fall hazard of approximately 10 feet by ensuring the ladder extended at least 3 feet above the upper landing surface to which the ladder is to gain access.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about March 2, 2015 during the installation of roofing materials on a residential home the employer did not ensure employees were protected from a fall hazard of up to approximately 10 feet by utilizing a fall protection system/device.  The employer, A&W Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(13), which was contained in OSHA inspection number 975874 citation number1 item number1a and was affirmed as a final order on May 16 2014, with respect to a workplace located at 5940 Graydon Rd. Seven Hills Ohio 44131.
