29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:   a. On or about January 22, 2016, during the installation of roofing materials on a newly constructed residential home located on lot 120 Stag Thicket, Brunswick, Ohio 44212, the employer did not ensure employees were protected from head injuries by requiring them to wear protective helmets when there was the possibility for head injury.  b. On or about January 22, 2016, during the installation of roofing materials on a newly constructed residential home located on lot 120 Stag Thicket, Brunswick, Ohio 44212, the employer did not ensure employees were protected from a fall hazard of approximately 10 feet by utilizing a fall protection system/device.  c. On or about January 22, 2016, during the installation of roofing materials on a newly constructed residential home located on lot 120 Stag Thicket, Brunswick, Ohio 44212, the employer did not ensure employees were protected from a fall hazard of approximately 10 feet by extending the ladder 3 feet over the roof landing area.
