29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required: (a) On or about June 3, 2015, during the installation of roofing materials, the employer did not ensure that employees working under other employees were protected from head injury by falling material by providing and requiring the use of protective helmets. (b) On or about June 3, 2015, during the installation of roofing materials at a newly constructed residential home, the employer did not ensure employees were protected from potential eye injuries while using air powered roofing nail guns. (c) On or about June 3, 2015, during the installation of roofing materials on a residential home the employer did not ensure employees were protected from a fall hazard of up to approximately 11.5 feet by utilizing a fall protection system/device. (d) On or about June 3, 2015, during the installation of roofing materials on a residential home, the employer did not ensure employees were protected from a fall hazard of approximately 11.5 feet by ensuring the ladder extended at least 3 feet above the upper landing surface to which the ladder is to gain access.
