29 CFR 1926.102(a)(1): Eye and face protective equipment are not used when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:     a. The employer fails to assure effective protection against eye and face injuries as required by the standard, as demonstrated on March 4, 2015, when employees are working on a residential roof at 12357 Boston Harbor Drive, Jacksonville Florida 32225 without eye protection while using pneumatic nail guns and were exposed to eye injuries from flying debris or nails.  Jasper Contractors, Inc., previously committed a serious violation of this standard on September 11, 2012, with respect to a workplace at 2927 Newbury Court, Highlands Ranch, CO, which became a final order on November 28, 2012 (Inspection No. 626158).        Jasper Contractors, Inc., previously committed a serious violation of this standard on July 25, 2012, with respect to a workplace at, 10775 Glengate Loop, Littleton, CO, which became a final order on November 28, 2012 (Inspection No. 546338).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels are not protected by guardrail systems, safety net system, or personal fall arrest system, nor are employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):      a. The employer fails to assure effective protection against falls as required by the standard, as demonstrated on March 4, 2015, when employees are working on a residential roof at 12357 Boston Harbor Drive, Jacksonville Florida 32225 without fall protection and were exposed to falls of approximately 13 feet.    Jasper Contractors, Inc., previously committed a serious violation of this standard on March 25, 2014, with respect to a workplace at 12983 Bent Water, Jacksonville, FL 32215, which became a final order on July 31, 2014, (Inspection No. 964742).
