29 CFR 1910.147(c)(2)(ii): An energy isolating device that was capable of being locked out, did not utilize a lockout device, which rendered the machine or equipment inoperative. On or about February 26, 2015, an employee engaged in sanitation activities at the Intestine Auger in the EEC Room was exposed to amputation hazards in that the auger was not deenergized and locked out prior to cleaning.
