29 CFR 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: A.) Hotel Construction Project/6310 Old Station Road, North East, PA/Roof Areas: On or about March 5, 2015, employees were exposed to a fall hazard of at least 20 to 40-feet while performing work operations associated with the installation of roof decking from the roof areas and trusses. The employees were wearing fall protection harnesses but were not attached to a fall protection anchorage point. Additional means of fall protection such as safety nets and guardrails were not utilized as a means of fall protection while working from the roof area.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: A.) Hotel Construction Project/6310 Old Station Road, North East, PA/Roof Areas: On or about March 5, 2015, employees were exposed to a fall hazard of at least 40-feet while working from the roof of the structure installing weather barrier material across the roof deck. The employees working on the roof were wearing a fall harness but were not attached to a fall protection anchorage point. Additional means of fall protection such as safety nets and guardrails were not utilized as a means of fall protection while working from the roof area.
