29 CFR 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: A.) Dynasty Construction Unlimited, Inc., 4500 Hammocks Drive, Erie, PA 16506, on or about April 10, 2015 and times prior thereto: Employees were installing floor decking on the third floor of the structure and were exposed to a fall hazard of 9.7-feet to the second floor below. The employees were not utilizing a means or method of fall protection including, but not limited to personal fall arrest systems, safety net systems, guardrail systems, or a fall protection plan. B.) Dynasty Construction Unlimited, Inc., 4500 Hammocks Drive, Erie, PA 16506, on or about April 10, 2015 and times prior thereto: Employees were installing floor decking on the third floor of the structure and were exposed to a fall hazard of 19.4-feet to the ground below. The employees were not utilizing a means or method of fall protection including, but not limited to personal fall arrest systems, safety net systems, guardrail systems, or a fall protection plan.

Dynasty Construction Unlimited, Inc., 4500 Hammocks Drive, Erie, PA 16506, second floor, on or about April 10, 2015 and times prior thereto: A flexible cord that was running from the power supply in an adjacent building was frayed at the cord cap. The cord was supplying power to electrically powered equipment including, but not limited to, a DeWalt radio and two handheld powered circular saws. B.) Dynasty Construction Unlimited, Inc., 4500 Hammocks Drive, Erie, PA 16506, second floor, on or about April 10, 2015 and times prior thereto: The flexible cord for a DeWalt radio was frayed at the termination into the radio.
