The employees were not utilizing a means or method of fall protection including, but not limited to personal fall arrest systems, safety net systems, guardrail systems, or a fall protection plan.    B.)  Dynasty Construction Unlimited, Inc., 4500 Hammocks Drive, Erie, PA  16506, on or about April 10, 2015 and times prior thereto:  Employees were installing floor decking on the third floor of the structure and were exposed to a fall hazard of 19.4-feet to the ground below.
