(b)  On or about December 18, 2014, at a worksite located at Pearl Road and State Route 82, Strongsville, OH 44136, during carpentry and framing activities, the employer did not perform or have a method for frequent and regular inspections to be conducted by a competent person.  (c)  On or about December 18, 2014, at a worksite located at Pearl Road and State Route 82, Strongsville, OH 44136, during carpentry and framing activities including but not limited to installing roof trusses from an elevated platform, the employer did not ensure the powered industrial vehicle was designed for such use.
