29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:  (a)  On or about May 28, 2015, during the installation of roofing materials at a job site located at 3536 Lincoln Way East, Massillon, Ohio 44646, the employer did not ensure that employees whom were working directly below other employees working on the roof were protected from head injury by providing and requiring the use of protective helmets.  (b)  On or about May 28, 2015, during the installation of roofing materials at a job site located at 3536 Lincoln Way East, Massillon, Ohio 44646, the employer did not ensure employees were protected from potential eye injuries while using reciprocating saw to cut roofing materials.  (c)  On or about May 28, 2015 at the job site located at 3536 Lincoln Way East, Massillon, Ohio 44646 during the installation of roofing material on a low slope roof, the employer did not ensure employees were protected from a fall of approximately 18.3 feet by utilizing a guardrail system, fall protection system/device, and/or safety monitor system.   (d)  On or about May 28, 2015 at the job site located at 3536 Lincoln way East, Massillon, Ohio 44646 during the installation of roofing material on a low slope roof, the employer did not ensure portable ladders extend 3 feet above the upper landing surface.
