29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.    On or about April 13, 2015, during the installation of roofing material at a job site located at 3536 Lincoln Way East, Massillon Ohio 44646 employees were observed utilizing the inside support ladder to gain access to the mounted ladder jack scaffold..

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   On or about April 13, 2015, during the installation of roofing material at a job site located at 3536 Lincoln Way East, Massillon Ohio 44646, the employer did not ensure employees working on a ladder jack scaffold were protected from a fall of approximately 13.4 feet by utilizing a fall protection system.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety net.   On or about April 13, 2015 at the job site located at 3536 Lincoln way East, Massillon, Ohio 44646 during the installation of roofing material on a low slope roof, the employer did not ensure employees were protected from a fall of approximately 18.3 feet by utilizing a guardrail system, fall protection system/device, and/or safety monitor system.
