29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function:    (a) On or about March 9, 2015, and at times prior, the employer did not ensure that employees working on a roof greater than 6 feet were protected from falls by the use of a guardrail, safety net, personal fall arrest system, or monitoring system.      Abatement note: Abatement certification and documentation are required for this item (See Certification of Corrective Action Worksheet).    D&D Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number 313720070, citation number 2,  item number 1 and was affirmed as a final order on 7/21/2010 with respect to a workplace located at 1801 S. Allison St, Lakewood, CO.    D&D Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number 313717951, citation number 1,  item number 1 and was affirmed as a final order on 4/13/2010 with respect to a workplace located at 1745 Allison St, Lakewood, CO.

29 CFR 1926.503(c)(3): When inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill, the employer did not retrain the affected employee:    a)  On or about March 9, 2015, employees were not retrained when they did not retain the necessary understanding and use of fall protection systems, including safety monitoring systems.
