29 CFR 1910.23(a)(4): Every skylight floor opening and hole shall be guarded by a standard skylight screen or a fixed standard railing on all exposed sides. On or about 3/9/2015 on the roof of the building located at 402 Blue Bird Blvd in Fort Valley, Georgia: Employees exposed to a fall hazard at heights of approximately 24 feet while working on the roof near an unguarded skylight. One employee stepped on the unguarded skylight and fell about 24 feet to the ground level. Every skylight floor opening and hole was not guarded as required.

They were also not of a construction and mounting that under ordinary loads or impacts, they would not deflect downward sufficiently to break the glass below them. On or about 3/9/2015 on the roof of the building located at 402 Blue Bird Blvd in Fort Valley, Georgia: Employees exposed to a fall hazard at heights of approximately 24 feet while working on the roof near a skylight that broke after an employee stepped on the skylight and fell to the lower level.

All ladders were not inspected regularly, with the intervals between inspections being determined by use and exposure. On or about 3/11/2015 on the roof of the building located at 402 Blue Bird Blvd in Fort Valley, Georgia: Metal fixed ladders on the roof and to the catwalk were missing bolts, broken, and not properly secured to prevent the employees from being exposed to a fall hazard.
