29 CFR 1910.333(a)(1): Live parts to which an employee was exposed were not deenergized before the employee worked on or near them. a) Plant - On or about 10/25/19 a worker was instructed to perform non-emergency electrical work on an energized 480 V plug-in breaker box on a busway. Or in the alternative: 29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized: a) Plant - On or about 10/25/19 a worker was instructed to perform non-emergency electrical work on an energized 480 V plug-in breaker box on a busway. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards did not use electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed: a) Plant - On or about 10/25/19 a worker was instructed to perform non-emergency electrical work on an energized 480 V plug-in breaker box on a busway, and the employer did not ensure that personal protective equipment was in use. On in the alternative: 29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) Plant - On or about 10/25/19 a worker was instructed perform non-emergency electrical work on an energized 480 V plug-in breaker box on a busway, and the employer did not ensure that personal protective equipment was in use. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
