Employees were exposed to amputation and heat hazards while all hazardous energy sources were not controlled prior to conducting work.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: A) Quality Blow Molding, Inc., located at 635 Oberlin Elyria Road, Elyria, OH 44035: On or about March 2, 2015, the employer did not ensure that employees utilize established energy control procedures to control hazardous energy of the B-13 Plastic Injection Blow Molding Machine # 1. Employees did not control all hazardous energy sources such as, but not limited to, electrical and hydraulic before performing activities such as, but not limited to, unjamming the machine. Employees were exposed to amputation and heat hazards. Quality Blow Molding, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.147(c)(1)), which was contained in OSHA Inspection # 313784936, Citation number 1, Item number 1, and was affirmed as a final order on December 20, 2010, with respect to a workplace located at 635 Oberlin Elyria Road, Elyria, OH 44035.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A) Quality Blow Molding, Inc., located at 635 Oberlin Elyria Road, Elyria, OH 44035: On or about March 2, 2015, the employer failed to ensure that all the steps outlined in 29 CFR 1910.147(d) and required for the control of the hazardous energy sources such as, but not limited to, electrical and hydraulic were followed before employees conducted activities such as, but not limited to, unjamming the B-13 Plastic Injection Blow Molding Machine # 1. Employees were exposed to amputation and heat hazards. Quality Blow Molding, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.147(d)(3) and 1910.147(d)(6)), which were contained in OSHA Inspection # 956867, Citation number 1, Item number 3a & Item number 3b, and were affirmed as a final order on December 20, 2010, with respect to a workplace located at 635 Oberlin Elyria Road, Elyria, OH 44035.

29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA. A) Quality Blow Molding, Inc., located at 635 Oberlin Elyria Road, Elyria, OH 44035: On or about March 2, 2015, the employer did not notify OSHA within twenty-four hours after an in-patient hospitalization and amputation injury occurred on B-13 plastic injection blow molding machine #1. The delay in reporting of the in-patient hospitalization and amputation injury to OSHA can result in the delay of the incident investigation and identification of hazards.
