29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. b. Quality Blow Molding, Inc. located in Elyria, OH: On or about September 23, 2015, an employee did not utilize lockout procedures to control the hazardous energy of the plastic blow molding press #17 when performing head cleaning operations. Quality Blow Molding, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.147(c)(1)), which was contained in OSHA inspection #313784936, citation number 1, item number 1, and was affirmed as a final order on December 20, 2010, with respect to a workplace located at 635 Oberlin Elyria Road, Elyria, OH 44035.

29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the following actions listed in and was not done in sequence as required by 29 CFR 1910.147 (d)(1)-(6): b. Quality Blow Molding, Inc. located in Elyria, OH: On or about September 23, 2015, the employer failed to ensure that all steps outlined in 29 CFR 1910.147 (d) and required for the control of hazardous energy sources such as but not limited to, thermal and hydraulic were followed for plastic blow molding machine #17 before employees conducted activities such as head cleaning operations. c. Quality Blow Molding, Inc. located in Elyria, OH: On or about August 25, 2015, the employer failed to ensure that all steps outlined in 29 CFR 1910.147 (d) and required for the control of a hazardous electrical energy source was followed for employees conducting cleaning activities on the Nelmor plastic grinder located next to plastic blow molding machine #13. The electrical disconnect was still able to be energized after the employee placed his lockout device on the electrical disconnect. Quality Blow Molding, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.147(d)(3) and 1910.147(d)(6)), which were contained in OSHA inspection #956867, Citation number 1, Item number 3a and Item number 3b, and was affirmed as a final order on May 27, 2014, with respect to a workplace located at 635 Oberlin Elyria Road, Elyria, OH 44035.
