29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: A) Quality Blow Molding, Inc., located at 635 Oberlin Elyria Road, Elyria, OH 44035: On or about February 18, 2015, the employer did not ensure that one or more methods of machine guarding were provided to protect employees from moving parts such as, but not limited to, the rotating chuck of equipment such as, but not limited to, the Ganesh horizontal lathe machine (Model GT-2060). Employees were exposed to struck-by and amputation hazards.
