29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:  Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard:      a)  Filter Press Brown Belt, on or about 3/13/15:  Open sided work platform was not equipped with a standard railing.  Employee exposed to a 5 foot 10 inch fall hazard.    b)  Filter Press Green Belt, on or about 3/13/15: Open sided work platform was not equipped with a standard railing.  Employee exposed to a 4 foot fall hazard.    c)  Department 44, Blow Off Booth, on or about 3/12/15:  Open sided floor at the outside corner of the blow off booth was not equipped with a standard railing.  Employee exposed to a 5 foot 6 inch fall hazard.       Victor Insulators, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.23(c)(1), which was contained in OSHA inspection number 314352204, citation number 1, item number 1a, and was affirmed as a final order on 8/082011, with respect to a workplace located at 280 Maple Ave., Victor, NY.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)  Distribution Department, on or about 3/12/15:  Line 2 Hot Press, rotating capper was not provided with a guard.    b) Bend Moment Test Area, on or about 3/13/15:  The bend moment tester was not provided with guarding over the roller shafts and nip points where the roller and ceramic insulator meet.      c)  Machine Shop, on or about 3/13/15:  Bridgeport milling machine was not equipped with a guard to cover the chuck and tool.    d)  Ceramic Lab, on or about 3/12/15:  Schold mixer was not provided with guarding around the rotating shaft.       e)  Cement Mix Area, on or about 3/13/15:  Two cement mixers were not provided with adequate guarding around the rotating shafts.    Victor Insulators, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.32(a)(2), which was contained in OSHA inspection number 314352204, citation number 1, item number 3, and was affirmed as a final order on 8/08/2011, with respect to a workplace located at 280 Maple Ave., Victor, NY.
