29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section   a)  Station Post 4 & 5, on or about 1/24/19:  Energy control procedures were not utilized to control hazardous energy when an employee was engaged in cleaning the large cantilever rollers.  As a result, the employee suffered a fractured forearm when the steel wool being used for cleaning caught on a powered roller pulling the employee's arm around roller.

29 CFR 1910.219(c)(2)(i): All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires.  a)  Large Post Glaze, on or about 2/8/19: Glaze machine was not provided with guarding to protect the operator from contacting the rotating shafts.  b)  Single Spindle Lathe Department, on or about 2/8/19 and 2/21/19:  Employees hand sanding parts on Lathes 1, 2, 6, 7, 8 and 9 were exposed to unguarded rotating shafts.  c)  Lathe Department, on or about 2/11/19:  Employees hand sanding parts on Lathe 12 & 14 were exposed to unguarded rotating shafts.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)  Station Post 4 & 5, on or about 1/24/19:  Large Cantilever was not equipped with guarding over the rotating rollers to protect the operators from contact during normal operations and when cleaning.  On 1/24/19 an employee suffered a fractured left forearm when the steel wool being used to clean the rollers caught and pulled the employee's arm into the rotating roller.  b) Cutoff Area, on or about 2/8/19:  LeBlond 16-Inch Lathe was not guarded to protect the operator from contacting rotating parts.  c)  Glazing, on or about 2/8/19:  Flush glaze machine, endless belt was not provided with guarding over the in-running nip points of the belt and roller.   Victor Insulators, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1046676, citation number 2, item number 2, and was affirmed as a final order on 07/28/15, with respect to a workplace located at 280 Maple Ave., Victor, NY.
