Floor hole was covered by a piece of plywood that was not secured to the floor to prevent displacement. An employee was exposed to a fall through the hole from eight floor to the seventh floor. On or about 03/11/2015. The Advanced Contracting Solutions, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.502(i)(3), which was contained in OSHA inspection number 976177, citation number 01, item number 2a and was affirmed as a final order on 01/09/2015, with respect to a workplace located at 1191 Boston Road Bronx NY 10456.
