On or about March 17, 2015, in the Production area, at the #902 Niehoff model 110A5 wire draw machine, the wire drawing drive shaft and wire spooling guide pulleys were not guarded to prevent the entry of arms and hands into the ingoing nip points and rotating parts, exposing employees to caught-in and amputation hazards.    b.
