On or about January 31, 2018, in the production area on southeast side, the employer did not provide safety guards to prevent operators from coming into contact with the rotating head when operating closing machine # 30, exposing employees to caught-in, crushing and amputation hazards.  b.  On or about January 31, 2018, in the production area on southeast side, the employer did not provide safety guards to prevent operators from coming into contact with the rotating head when operating stranding machine # 31, exposing employees to caught-in, crushing and amputation hazards.  c.  On or about January 31, 2018, in the production area on the northwest side, the employer did not provide safety guards to prevent operators from coming into contact with the ingoing nip points on pulleys while operating drawing machine # 925, exposing employees to caught-in, crushing hazards and other disabling injuries.  d.  On or about January 31, 2018, in the production area shop on the north side, the employer did not provide safety guards to prevent operators from coming into contact with the ingoing nip points on pulleys, take-up and pay-off rolls while operating stranding machines # 103 and 109, exposing employees to caught-in, crushing hazards and other disabling injuries.
