29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces:     (a) Mando Corporation- Production: The employer failed to label and/or identify permit required confined spaces at the facility  for employees performing a variety of assigned duties involving foundry operations. Employees were exposed to oxygen deficient atmosphere, heat stress, engulfment, suffocation and other hazards associated with confined space entry such as but not limited to, sand mixer, silos, various dust collectors, and furnaces.

29 CFR 1910.146(d)(5)(i): The employer did not test conditions in the permit space to determine if acceptable entry conditions existed before entry was authorized:     a) Mando Corporation: The employer failed to ensure atmospheric testing was performed in spaces to determine the sufficiency of oxygen and the absence of other hazards for workers required to enter the confined spaces. Employees were exposed to inhalation hazards, oxygen deficient atmospheres, engulfment and other hazards while entering mixers, furnaces and other confined spaces to perform repairs, maintenance, cleaning and other activities.

29 CFR 1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit:     a) Mando Corporation: The employer failed to ensure that confined space permits were prepared and completed prior to employees entry into mixers, furnaces and other confined spaces to ensure all precautionary measures were completed prior to the employee entry. Employees were exposed to heat stress, oxygen deficient atmosphere, inhalation hazards, engulfment, suffocation and other injuries while cleaning, inspecting, performing maintenance and other assigned task without preparation of confined space permits.

29 CFR 1910.146(g)(1): The employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146 (permit required confined spaces) acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under 29 CFR 1910.146:      a) Mando Corporation: The employer failed to ensure that training on permit confined space entry was provided and  implemented for all employees participating in confined space entry activities. Employees were exposed to inhalation hazards, oxygen deficient atmospheres, engulfment and other hazards while entering mixers, furnaces and other confined spaces to clean, repair, maintenance, inspect and other assigned duties.
