29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:  a)  The employer did not provide documentation and certification that Citation 1 Item 1, Citation 1 Item 2, and Citation 2 Item 1 were abated.  These citations were issued for modifications done to forklifts, unguarded point of operation on a press brake, and forklift operator training respectively.

Leo Agriculture, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(1)(I), which was contained in OSHA inspection number 793362, citation number 1, item number 5 and was affirmed as a final order on 9/30/13, with respect to a workplace located at W5370 Hwy DR Monroe, WI 53566.
