The metal conduit containing flexible wiring on the Bridgeport drill was broken.  Employees were not trained on physical and health hazards related to the chemicals they work with.  The mezzanine was not marked with the approved load design.   Employees were not provided with protective eyewear.

Leo Agriculture was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1200(h)(1), which was contained in OSHA inspection number 793362, citation number 1, item number 10 and was affirmed as a final order on 9/30/2013, with respect to a workplace located at W5370 Hwy DR, Monroe, WI 53566.
