29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, were not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: On the ground around the exterior perimeter of the house, debris was not kept clear as there was lumber with nails that protruded about 0.75 inches, broken CMUs, broken pallets, pieces of conduit, and empty cardboard boxes, exposing employees using the area to work and access the house to the hazards of falling onto the debris.

29 CFR 1926.452(a)(3): Diagonal bracing in both directions was not installed across the entire inside face of double-pole scaffolds used to support loads equivalent to a uniformly distributed load of 50 pounds (222 kg) or more per square foot (929 square cm): Around the 42 by 36 foot perimeter of the house, there was no diagonal bracing on the inside set of poles, exposing employees to the hazard of falling 13.5 feet from the scaffold platform to the ground below.
