29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:  Awnex, Inc., Ball Ground, GA - In zones 8 and 2 , employees cutting metal and building special projects were exposed to noise in excess of 85 dBA. Initial noise monitoring was not conducted, on or about 3/25/15.

29 CFR 1910.95(k)(1): The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k). The employer did not institute a training program and ensure employee participation in the program:  a) Awnex, Inc., Ball Ground, GA - In zone 2, an employee building special projects was exposed to noise levels at TWA of 93 dBA. The sample was taken for 245 minutes period. Zero was assumed for the 235 minutes not sampled. A noise training program was not instituted, on or about 4/1/15.   b) Awnex, Inc., Ball Ground, GA - In zone 8, an employee cutting metal on the chop saw was exposed to noise levels at TWA of 87.7 dBA. The sample was taken for 426 minutes period. Zero was assumed for the 54 minutes not sampled. A noise training program was not instituted, on or about 4/1/15.
