29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment (PPE).  If such hazards were present, or likely to be present, the employer did not: (i) Select, and have each affected employee use, the types of PPE that would protect the affected employee from the hazards identified in the hazard assessment; (ii) Communicate selection decisions to each affected employee; and (iii) Select PPE that properly fit each affected employee:  a) Fabrication Area Zone 4 - On or about November 18,2021 and at times prior to, employees were exposed to electrical and thermal hazards. The employer did not conduct an assessment to determine the appropriate personal protective equipment for use while employee(s) were engaged in welding activities.

(a) Fabrication Area Zone 4 -  On or about  November 18,2021 and at times prior, employees were exposed to electrical shock hazard in that the employer did not ensure that appropriate hand protection was provided while in the proximity of heat and energized parts.

29 CFR  1910.252(b)(3): Protective clothing - General requirements. Employees exposed to the hazards created by welding, cutting, or brazing operations shall be protected by personal protective equipment in accordance with the requirements of 1910.132. Appropriate protective clothing required for any welding operation will vary with the size, nature and location of the work to be performed.   (a) Fabrication Area Zone 4 -  On or about  November 18,2021 and at times prior, employees were exposed to electrical shock hazard in that the employer did not ensure that appropriate protective clothing was provided while in the proximity of heat and energized parts.

29 CFR  1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.   a. Main Office - The employer did not notify OSHA of an in-patient hospitalization/electrical shock within 24 hours of being admitted. The employee was admitted as an in-patient on November 18, 2021, that was not reported to OSHA until December 8, 2021.
