29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Location - Plummer Avenue, Newburyport, MA: On or about 3/30/15, the employer failed to ensure that employees performing roofing work were provided with fall protection. Family Restoration, Inc. was previously cited for violations of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 316312206, citation number 1, item number 1, and in OSHA inspection number 487419, citation number 2, item number 1, and were affirmed as a final order on 3/20/12 and 8/20/12 respectively, with regard to workplaces located at 60 Bacon Street, Winchester, MA and 1 Glenwood Street, Burlington, MA.
