(a) Location - 19 Prince Avenue, Winchester, Massachusetts: On or about August 18, 2016 an employee performing roofing 11 feet 5 inches above the ground was not protected from falling to the ground below. Family Restoration, Incorporated was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection numbers, 316312206, 487419 and 1050393, citation numbers 01, 02 and 01, item numbers001, 001 and 001 and was affirmed as a final order on 3/20/2012, 10/19/2012 and 7/20/2015, with respect to workplaces located at 60 Bacon Street, Winchester, Massachusetts, 1 Glenwood Street, Burlington, Massachusetts and 2 Plummer Avenue, Newburyport, Massachusetts.
