29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) Drill 1212; On or about March 26, 2015 and at times prior thereto, the employer exposed employees to caught in hazards in that the employer did not ensure employees who were setting up drills locked out the equipment prior to performing the task. b) Paint line; On or about March 26, 2015 and at times prior thereto, the employer exposed employees to caught in and amputation hazards in that the employer did not ensure employees who were tasked with cleaning the paint line conveyor locked out the equipment prior to performing the job.

29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing, or blocking of machines or equipment from energy sources: a) Drill 1212; On or about March 26, 2015 and at times prior thereto, the employer exposed employees to caught in hazards in that the employer had not provided locks for employees to use to lock out Drill #2 so the machine could not be accidentally started when employees were setting up/adjusting the equipment. b) Paint line; On or about March 26, 2015 and at times prior thereto, the employer exposed employees to caught in and amputation hazards in that the employer had not provided locks for employees to use to lock out the paint line conveyor so the machine could not be accidentally started when employees clean the paint off the chains. Fras-Le North America, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(5)(I) which was contained in OSHA inspection number 964136, citation number 1, item number 4 and was affirmed as a final order on October 8, 2014, with respect to a workplace located at 103 Echlin Blvd., Prattville, Alabama.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: a) Drill 1212 (Drill #2); On or about March 26, 2015 and at times prior thereto, the employer failed to ensure an employee was trained and had acquired the knowledge and skills required to control energy sources such as but not limited to 480V electrical and compressed air pressure when setting up Drill #2. b) Paint line conveyor; On or about March 26, 2015 and at times prior thereto, the employer failed to ensure an employee was trained and had acquired the knowledge and skills required to control energy sources such as but not limited to 480V electrical and compressed air pressure when cleaning the paint from the chains on the paint line conveyor. Fras-Le North America, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(5)(I) which was contained in OSHA inspection number 964136, citation number 1, item number 5 and was affirmed as a final order on October 8, 2014, with respect to a workplace located at 103 Echlin Blvd., Prattville, Alabama.
