29 CFR 1910.147(f)(1)(ii): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2): a) Production Floor; On or about May 12, 2017 the employer exposed employees to amputation hazards in that employees were required to troubleshoot - test or position - IAG #2 while the machine was energized. The employer failed to follow the required sequence of actions as set by the standard when equipment is energized for testing or positioning. Employees were not removed from the machine or equipment area prior to the machine being energized.
