29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4):    Along the production floor, on or about April 02, 2015, and at times prior thereto employees were exposed to caught-in hazards while doing adjustments during the production of laminating wrapping paper without having lock-out tag-out training on a shop-made slitter machine.

No. 00C442L controlling the electrical power various outlets, shop wall fans and lighting units.      b) Along the center west section of the production floor, the fixed power enclosure EATON Cutler-Hammer No Serial Number controlling the electrical power of the water pump, paint pump, various outlets, laminating machine, extractor and lighting units.
