Columbus Ave., Springfield, MA: On or about March 31, 2015, an employee was sheathing the roof of this building more than 7 feet above the lower roof level without any fall protection. Arlen Company Construction LLC was previously cited for a violation of an equivalent standard 29 CFR 1926.501 (b) (1)which was contained in OSHA inspection number 966472 citation number 1, item number 1 and was affirmed as a final order on August 8, 2014 with respect to a workplace located at 183 Summer St., Kingston, MA.
