29 CFR 1926.304(h)(1): Each circular crosscut table saw was not guarded by a hood which completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut: Location: site On or abut 4/22/16 employees were exposed to an amputation hazard while using a Bosh table saw with the guard over the blade missing.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: Location: outside the building On or about 4/22/16 employees operating a Bosh table saw were using an extension cord which had the insulation pulled back around the plug area exposing the conductors and causing pull to joints or terminal screws.
