29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a ridgid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the ladder: On or about 3/17/2015, at the above addressed jobsite, employees were climbing an extension ladder which was not secured at the top to prevent displacement, exposing the employees to a 10 foot fall hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about 3/17/2015, at the above addressed jobsite, employees were engaged in residential roof work at approximately 10 feet above ground level without a means of fall protection. Coast 2 Coast Support, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 902103 citation number 1, item number 1, and was affirmed as a final order on 5/24/2013 with respect to a workplace located at 12950 Vista Isles Dr. Plantation, FL 33325.
