29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, were not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:      Near the north wall of the project, debris was not kept cleared from work areas and passageways as there were steel studs, broken drywall, pieces of conduit, and pieces of lumber on the floor, exposing employees to the hazards of falling to the same level on the debris.

In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy:    (a.) Near the north wall of the project, there was no cover on a junction box designed for a receptacle.  The energized wires in the box were spliced to a nonmetallic sheathed cable that supplied power to a receptacle in an electrical box that lay on the floor.  Employees, applying joint compound on the drywall, troweling concrete on the floor, and moving tiles and other supplies near the fixtures, were exposed to the hazards of contacting electricity.  (b.) Near the north wall of the project, there was no cover or faceplate on the outlet box that lay on the floor with an energized 110 VAC outlet. These requirements are in addition to any other requirements for equipment grounding conductors:      Near the south wall of the project, there were no ground fault circuit interrupters or assured equipment grounding conductor program to protect employees that used an extension cord to power a 110 VAC Makita FS4200 screw gun to attach drywall.  Employees were exposed to the hazard of contacting electricity.

Employees, applying joint compound on the drywall, troweling concrete on the floor, and moving tiles and other supplies near the fixtures, were exposed to the hazards of contacting electricity.    (b.) Near the north wall of the project, the splices of nonmetallic sheathed cable (Romex) were not guarded against accidental contact by cabinets or enclosures.  The cable was spliced to the energized wires from an electric box on the wall.  Employees, applying joint compound on the drywall, troweling concrete on the floor, and moving tiles and other supplies near the fixtures, were exposed to the hazards of contacting electricity.
