Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazards: a. On or about April 3, 2015, at Suite 300 of the above addressed jobsite, an employee was exposed to a fall hazard while installing an acoustical ceiling from stilts on a recently waxed floor.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a. On or about April 3, 2015, at the above addressed jobsite, an employee was hospitalized after falling from stilts while installing an acoustical ceiling. The incident was reported on April 6, 2015.
