29 CFR 1926.404(b)(1)(i): General. The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors. (a) At the project site, on about April 8, 2015, the employer failed to provide ground fault circuit interrupters for employee that used RIDGIT Pipe Threading Machine, Model A, No. 700, Serial Number 1077827. The machine was not designed double insulated, exposing workers to the hazards of electrical shock.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords were not connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. (a) At the project on or about April 8, 2015, and at times prior thereto, no strain relief was provided for flexible orange power cord that was plugged into a wall outlet receptacle. The cord carried an electrical current to RIDGIT Pipe Threading Machine, Model A, No. 700, Serial Number 1077827, exposing workers to the hazards of electrical shock.
