29 CFR 1910.1200(h)(3)(i): Employee training did not include methods and observations used to detect the presence or release of a hazardous chemical in the work area (such as monitoring conducted by the employer, continuous monitoring devices, visual appearance or odor of hazardous chemicals when being released, etc.): (a) On or about March 7, 2015, for the employees exposed to severe burn injuries while working on and around the process equipment where crude oil vapors ignited resulting in severe damage of six waste mud/oil storage tanks and the shaker system. Employees were not properly trained and knowledgeable in reading, operating, and reporting of the 4-gas monitor (O2, H2S, CO, LEL), at Tervita, LLC Alexander Treatment Recovery Disposal (TRD) facility located at 14391 39th Street NW in Alexander, North Dakota.
