29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports: a) 9th & Wharton St, Philadelphia, PA - The employees were working off of a fabricated frame scaffold approximately 21 feet long, 6.5 feet high and 5 feet wide which was not fully planked or decked on all working levels between the front uprights and the guardrail supports thus exposing the employees to a fall hazard. This condition was observed on April 7, 2015.

29 CFR 1926.451(e)(1): Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers or stairway-type ladders was not used for access: a) 9th & Wharton St, Philadelphia, PA - The employees were working off of a fabricated frame scaffold approximately 21 feet long, 6.5 feet high and 5 feet wide which did not have proper access for the employees to reach the working level of the scaffold. This hazard exposed the employees to a fall hazard. This condition was observed on April 7, 2015.
