29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a) Jobsite: 3344 N 22nd Street, Philadelphia, PA 19140 - On or about August 26, 2022, the employer did not ensure that employees conducting residential demolition operations activities 6 feet or more above lower levels were protected by guardrail systems, safety net system, or personal fall arrest system. Employees were demolishing a residential dwelling from the roof of a three-story building with the use of a fall protection system, thereby exposing employees to falls approximately 30 feet from the ground.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Jobsite: 3344 N 22nd Street, Philadelphia, PA 19140 - On or about August 26, 2022, The employer did not provide fall protection training to include information on how to recognize, evaluate and mitigate fall hazards to employees exposed to 30 foot falls while engaged in demolition activities on a residential building.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:  a) Jobsite: 3344 N 22nd Street, Philadelphia, PA 19140 - On or about August 26, 2022, the employer did not ensure that side rails of portable ladders extended at least 3 feet above the upper landing surface to which the ladder was used to gain access. Employees conducting residential demolition operations utilized a portable ladder that did not extend at least 3 feet above the landing surface to access the third story of a residential building, thereby exposing employees to fall hazards.
