Employees were exposed to crushing injury in that the electrical/hydraulic energy source was not isolated.

Haviland Plastic Products, Co. - Haviland, Ohio: On or about February 2, 2015, the employer did not ensure that the Yale forklift (s/n N522699), used to lift a mold during the press #18 changeover, was marked to include the boom front-end attachment showing the capacity and approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. A potential for the forklift to be tipped over as a result of being overloaded exposed employees to crushing hazards.
