29 CFR 1910.303(b)(1)(ii):  Electric equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees. The safety of equipment was not determined by considering  the mechanical strength and durability, including, for parts designed to enclose and protect other equipment, and the adequacy of the protection thus provided.  a. Haviland Plastic Products, Co. located in Haviland, Ohio:  On or about October 19, 2015, the employer did not ensure wire nuts used to splice 480 volt conductors for the heater bands on plastic blow molding press #18, located in the production department, were enclosed in an approved box at each splice point.  Abatement Note: One feasible and acceptable method to correct this hazard is to comply with National Fire Protection Association (NFPA) Standard 70, National Electric Code 2014 Ed. - Section 300, and or comply with Tempco Electric Heater Corporation engineering data specification that states all wiring needs to comply with the National Electrical Code.

Haviland Plastic Products, Co. located in Haviland, Ohio:  On or about October 19, 2015, junction boxes for the heater band electrical supply conductors on plastic blow molding press #18, located in the production department, were not equipped with covers, exposing employees to accidental contact with the energized conductor terminals.

29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertain to their respective job assignments.    a. Haviland Plastic Products, Co. located in Haviland, Ohio:  On or about September 26, 2015, an employee was not properly trained on electrical protective equipment to be worn and electrical shock hazards prior to being exposed to energized electrical equipment when reaching into the 480 volt electrical panel for plastic blow molding press #18 to activate a reset switch.    b. Haviland Plastic Products, Co. located in Haviland, Ohio:  On or about October 5, 2015, an employee was not properly trained on the proper electrical protective equipment to be worn and proper inspection of electrical testing equipment prior to troubleshooting a contactor inside of the 480 volt electrical panel for plastic blow molding press #1.
