29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Coater line # 6, Coater head # 1; Energy isolation devices were not used during servicing and maintenance operations, such as but not limited to setting up, cleaning, and clearing jams. Loparex, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d)(4)(i), which was contained in OSHA inspection number 96786, citation number 1, item number 4b and was affirmed as a final order on 12/17/2012, with respect to a workplace located at 1740 Ridgeway Street, Hammond, WI.
