29 CFR 1926.404(b)(1)(i): General.  The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors.  (a) At the Pia Marine Hotel project, the employer failed to provide ground fault circuit interrupters for employee that used Ridgid cut off saw and Makita square sander inside a building. The tools were connected to a wall outlet receptacle missing a cover face. Employees were exposed to the hazards of electrical shock.

29 CFR 1926.405(b)(1): Unused openings in cabinets, boxes, and fittings were not also effectively closed:   (a) At the Pia Marine Hotel project, one 110 VAC wall outlet receptacle box was not provided with a face cover plate and the exposed terminals were energized.  The box was located less than 24 inches above the walking and working floor. Employees were exposed to possible accidental contact with exposed live parts and receive an electrical burn and shock.

29 CFR 1926.405(g)(2)(iv): Strain relief.  Flexible cords were not connected to devices and fittings so that strain relief was provided which will prevent pull from being directly transmitted to joints or terminal screws.  (a) At the Pia Marine Hotel project, the outside insulation on an extension cord was not connected to the plug that was used to power two portable Ridgid cut off saw and Makita square sander, exposing employees to the hazard of contacting live electrical wires.

29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, were not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  (a) At the Pia Marine Hotel project, lumber with protruding nails and other debris were not removed from walking and working surface inside the project site. Employees were exposed to stepping on the nails.

The tools were powered through an extension cord missing the prong.  The cord was plugged into an interior outlet.  Employee was exposed to the hazards of electrical shock.
