29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. Frozen Specialties, Inc. - Archbold, OH: On or about April 21, 2015, shaft ends that projected from the North Line processing conveyor belt were not guarded. Employees were exposed to rotating part hazards when making tension adjustments to the conveyor belt. b. Frozen Specialties, Inc. - Archbold, OH: On or about April 16, 2016, the employer did not ensure that employees were protected from the in-running nip points created where the north line processing transfer conveyor belt passes under a horizontal rotating shaft at the beginning of the transfer and again when the belt passes over a horizontal rotating shaft.
